The Fatigue Assessment Scale (FAS) is © copyright of the ild care foundation in all languages.

The use of the Fatigue Assessment Scale (FAS) is permitted by the ild care foundation under the following conditions:

• Usage in clinical settings is permitted without restrictions.
• Translations can only be undertaken by the ild care foundation, or with their permission, and must be performed under strict conditions.*
• Usage in clinical research is also permitted. In this case, the ild care foundation must be acknowledged in the manuscript as follows: 
  © FAS (Fatigue Assessment Scale): ild care foundation (www.ildcare.nl), and “Fatigue Assessment Scale (FAS)” must be included as a keyword in the final publication.
• Use of the FAS for research initiated or financially supported by pharmaceutical companies, industry, or institutes requires a license. A one-off fee of €500 per questionnaire per language applies. For further information, please contact: info@ildcare.nl.
• A PDF file of the final publication must be sent to the ild care foundation: info@ildcare.nl.
• For any company or healthcare setting that wants to use the FAS (or adaptations) for commercial purposes, the ild care foundation requires a one-off fee of €500 per questionnaire/translation to compensate for development costs. For further information, please contact: info@ildcare.nl

*If a translation is required in another language, the following conditions must be met:

• Copyright remains with the ild care foundation. The final validated translation must be provided to the ild care foundation.
• A validated forward–backward–forward translation method must be used.
• New translations will only be made available via the ild care foundation website (ildcare.nl).
• The PDF must follow the ild care foundation  (see website version: layout).
• No costs may be charged to the ild care foundation or its collaborators for the translation work.
