Fysieke training bij sarcoïdosepatiënten met vermoeidheidsklachten

Het klinisch beloop van sarcoïdose is zeer variabel, waarbij vrijwel elk orgaan betrokken kan zijn. Bij 90% van de sarcoïdosepatiënten zijn de longen aangedaan, maar oog- en huidproblemen worden ook veelvuldig gerapporteerd. Echter inspanningsintolerantie, spierzwakte en vermoeidheid zijn tevens veel voorkomende klachten met een persisterend karakter bij patiënten met sarcoïdose. Meerdere studies hebben aangetoond dat de kwaliteit van leven (KvL), onder andere als gevolg van fysieke beperkingen en vermoeidheid, van sarcoïdosepatiënten verminderd is. Patiënten met dergelijke problematiek zouden mogelijk baat kunnen hebben bij een trainingsprogramma. Tot op heden ontbreekt wetenschappelijk onderzoek naar de effecten van een trainingsprogramma op de functionele beperkingen van sarcoïdosepatiënten.

Het doel van deze studie met als titel ‘Fysieke training bij sarcoïdosepatiënten met vermoeidheidsklachten’ is dan ook om te onderzoeken of een fysiotherapeutisch trainingsprogramma van invloed is op de mate van vermoeidheid, fysieke functies en KvL. De onderzoeksvraag luidt: ‘Leidt een 13-weken durend trainingsprogramma tot een klinische verbetering van vermoeidheid, fysieke functies en KvL bij sarcoïdosepatiënten?

Uitsluitend volwassen sarcoïdosepatiënten die voor een trainingsprogramma door de longarts zijn verwezen naar fysiotherapiepraktijk FysioMedica (www.fysiomedica.nl) te Geleen in verband met vermoeidheidsklachten en/of inspanningsintolerantie worden benaderd voor deelname aan dit onderzoek. De inclusie van 40 patiënten vindt plaats gedurende de periode januari 2013 tot en met juli 2013. Patiënten die het afgelopen jaar reeds hebben deelgenomen aan een trainingsprogramma worden geëxcludeerd.

Sarcoïdosepatiënten zullen onder fysiotherapeutische begeleiding gedurende 13 weken, drie maal per week 1 uur, deelnemen aan kracht- (been- en armspieren) en conditietraining (fietsergometrie en loopband).

Voorafgaande aan, na afloop en 13 weken na de stopzetting van het trainingsprogramma worden de patiënten fysiek in kaart gebracht middels: 1) vragenlijsten voor het bepalen van de mate van vermoeidheid (Fatigue Assessment Scale) en de kwaliteit van leven (WHOQOL-Bref), 2) perifere spierkrachttesten voor de bovenbeenspieren (m. quadriceps) en armspieren (m. biceps) en 3) inspanningstesten (6-minuten wandel test en submaximale fietstest).

Middels een statistische analyse wordt de verandering in de tijd van de hierboven benoemde onderzoeksparameters beschreven. Getracht zal worden om relevante bevindingen te verwerken in de behandeling, de therapeutische strategieën en de multidisciplinaire benadering van de sarcoïdosepatiënt.

Onderzoeker

Rik Marcellis (1981) heeft in 2005 de HBO opleiding fysiotherapie aan de Hogeschool Zuyd te Heerlen afgerond en in 2006 de studie Bewegingswetenschappen aan de Universiteit Maastricht te Maastricht. Hij is sinds mei 2006 werkzaam als fysiotherapeut bij de afdeling fysiotherapie van het Maastricht Universitair Medisch Centrum (MUMC) met als specialisme neurologie/neurochirurgie. Tevens is hij als fysiotherapeut verbonden aan het multidisciplinaire Cystic Fibrosis team van het MUMC. Momenteel doet hij onderzoek naar de prevalentie van perifere spierzwakte en inspanningstolerantie bij patiënten met sarcoïdose en de relatie tussen deze symptomen enerzijds en de kwaliteit van leven en overige klinische parameters anderzijds. e-mail: rik.marcellis@mumc.nl.

Drs. Rik Marcellis (e-mail: rik.marcellis@mumc.nl; links), fysiotherapeut in het Maastricht Universitair Medisch Centrum (MUMC+) en onderzoeker van het voormalige ild care team o.l.v. prof. dr. Marjolein Drent heeft voor zijn onderzoek een prijs gewonnen tijdens een internationaal congres. Hij heeft aangetoond dat longfunctieonderzoek alleen bij sarcoïdosepatiënten met een normale diffusie capaciteit niet uitsluit dat er toch problemen kunnen zijn met de opname van zuurstof.

Van 15 tot en met 18 juni is in Maastricht de wereldtop op het gebied van sarcoïdose bijeengekomen tijdens het 10e WASOG congres. De WASOG is een wereldorganisatie die zich bezighoudt met sarcoïdose en andere diffuse ofwel interstitiële longaandoeningen. Sarcoïdose is een hele grillige aandoening. Het komt vooral bij relatief jonge mensen voor. Het is een stoornis van je afweersysteem. Ieder deel van het lichaam kan betrokken raken bij de ziekte, maar het komt het meeste in de longen voor. Het belangrijkste probleem is een enorme verlammende alles beheersende moeheid. Dit heeft regelmatig tot gevolg dat de betrokken niet goed meer in de maatschappij kan functioneren en gedeeltelijk of zelfs helemaal arbeidsongeschikt wordt. Er waren 100 wetenschappelijke abstracts opgestuurd. Uit deze 100 abstract zijn acht inzendingen uitgekozen door een internationaal panel van experts en deze onderzoekers mochten de resultaten tijdens een sessie op donderdagmiddag 17 juni in het MECC presenteren. De voorzitters van de sessie en tevens de jury waren Prof. Roland M. du Bois, hoogleraar longziekten aan het Imperial College in Londen en Dr. Wonder P. Drake, Vanderbilt University School of Medicine, uit Nashville, Tennessee, USA. Zij hebben uiteindelijk Rik Marcellis uitgekozen vanwege zijn uitstekende presentatie en hebben hem persoonlijk de prijs overhandigd tijdens het congresfeest in de mooiste boekhandel van de wereld: Selexyz Dominicanen.

De titel van zijn onderzoek was: Verminderde gasuitwisseling, inspanningstolerantie en spierkracht in sarcoïdosepatiënten (‘Impaired gas exchange, exercise capacity and muscle strength in sarcoidosis’). Van de 160 onderzochte sarcoïdosepatiënten bleken er 62% een normale diffusiecapaciteit (DLCO) te hebben. Van deze patiënten had 70% een probleem met zuurstofopname tijdens inspanning (fietstest), waarvan bijna 20% een ernstig probleem bleek te hebben. De conclusie van zijn onderzoek is dan ook dat als een patiënt moeite heeft met inspanning en snel uitgeput is, dat het dan goed is om aanvullend een inspanningstest te doen om een zuurstofopnameprobleem uit te sluiten ook al is de longfunctie normaal. Dit kan bovendien helpen bij het vaststellen van de ernst van sarcoïdose.

De prijs is een iPad, bedoelt als stimulans om nog flexibeler aan het onderzoek te kunnen werken. De iPad is ter beschikking gesteld door: ild care foundation en de Foundation for Sarcoidosis Research USA.

 

Optimization of pharmacotherapeutic options in sarcoidosis

The clinical presentation and course of sarcoidosis are highly variable, varying from minimal involvement to derangement of organ physiology with functional impairment. Most sarcoidosis patients show spontaneous resolution, but in patients with a severe disease course and poor prognosis, a timely implementation of an appropriate potent individual pharmacological treatment regimen is important. Despite the availability of several immunosuppressive therapeutic options in sarcoidosis, there is a lack of standardized management strategies. The published data on the different pharmacological therapeutics are limited. The studies performed in this research project evaluate several aspects of therapeutic options in sarcoidosis with the aim of improving the management of patients who require pharmacological treatment.

This research project was started with the evaluation of the extent, distribution and consistency of organ involvement detected by fluorine18-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT or PET) in sarcoidosis patients with persistent disabling symptoms. Positive PET findings were classified as thoracic and/or extrathoracic. Involvement of individual organs was assessed as well.

The next project investigates the prevalence of muscle atrophy and cachexia in Dutch sarcoidosis patients, and the association of these body composition profiles with sarcoidosis disease activity and severity. Fat-free mass was assessed as an indirect measure of muscle mass by bioelectrical impedance analysis. Patients were stratified based on body mass index (BMI) and fat-free mass index (FFMI). The aim was to establish if muscle atrophy and cachexia are frequent problems in sarcoidosis which should be taken into account in disease management.

The third part of the research project consists of the optimization of management of hepatic sarcoidosis and handling of liver-test abnormalities. Patients with hepatic involvement are usually asymptomatic, but in a significant number disabling symptoms can be present. Furthermore, sometimes hepatic sarcoidosis can be a rapid progressive disease with the occurrence of complications, such as liver cirrhosis. Diagnosis is difficult, as studies are scarce, diagnostic criteria are lacking and guidelines with recommendations for diagnosis and follow-up of patients suspected of hepatic sarcoidosis are not available. The presence and severity of liver-test abnormalities in a large cohort of patients with confirmed sarcoidosis was established. Additionally, the association between severity of liver-test abnormalities and histopathological abnormalities in hepatic sarcoidosis was evaluated. Furthermore, an overview of the knowledge available in the literature about therapeutic options for sarcoidosis liver involvement was given. The available data were combined into recommendations for the optimal therapeutic approach in clinical practice.

The development of practical recommendations, on behalf of the World Association of Sarcoidosis and Other Granulomatous Disorders (WASOG), for the use of methotrexate (MTX) in sarcoidosis by integrating the evidence obtained through a systematic literature review and the opinions of sarcoidosis experts worldwide, is presented in the next project.

Practical recommendations are also developed for the use of the anti-TNF-α agents infliximab and adalimumab in sarcoidosis, by combining the evidence from both a systematic literature search and the expert opinions of the world’s leading sarcoidologists worldwide.

And finally, possible factors identifying responders to anti-TNF-α agents are explored. The association between the presence of the TNF-α G-308A polymorphism and the response to anti-TNF-α therapy amongst patients with refractory sarcoidosis was assessed.

Anne Cremers (1985) studied medicine at the Maastricht University, where she obtained her MD in 2010. In 2010 she started her training in internal medicine at the Atrium Medical Centre Heerlen, the Netherlands. In 2014 she will start her training in rheumatology at the Rijnstate Hospital in Arnhem and the University Medical Centre St Radboud Nijmegen, the Netherlands. In 2010 she started research activities in sarcoidosis at the Maastricht University Medical Centre+ (MUMC+), NL. At the 2010 meeting of the Dutch organization of Gastroenterology and Hepatology, and the WASOG/BAL conference in June 2011 in Maastricht, The Netherlands, she presented the study on liver-test abnormalities in sarcoidosis in an oral presentation and a poster, respectively. During the WASOG meeting in June 2013 in Paris, France, she presented the results of the study ‘Body composition profiling in sarcoidosis’ during a poster presentation.
