Fatigue Assessment Scale: © Copyright, in any language, belongs to the ild care foundation

The use of the Fatigue Assessment Scale (FAS) is permitted by the ild care foundation,
according to the following conditions:

1. Usage in clinical settings is permitted without any condition
2. Translation of this questionnaire can only be done by the ild care foundation, or by their permission and must be done under strict conditions.*
3. Usage in clinical research is also permitted, in this case you must acknowledge the ild care foundation as follows, by adding the following sentence to the manuscript: © FAS (Fatigue Assessment Scale): ild care foundation (www.ildcare.nl), and the Fatigue Assessment Scale (FAS) needs to be added as a keyword in the final publication.
4. Usage of the FAS for research initiated or financially supported by Pharmaceutical Companies, or any other Industry, or Institutes.
   Costs on-off € 500,00 per questionnaire, per language. For further information please, send an email to info@ildcare.nl
5. A PDF file of the final publication must be sent to the ild care foundation: info@ildcare.nl
6. For any company or healthcare setting that wants to use the FAS (or adaptations) for commercial purposes, the ild care foundation asks a one-off amount of € 500,00 per questionnaire / translation to compensate for development costs. For further information please, send an email to info@ildcare.nl

*If you would need a translation in any language than the following conditions must be taken into account:

1. Copyright is, and always will be with ild care foundation, so the final validated translation will be made available to the ild care foundation.
2. You will use a validated forward-backward-forward translation method.
3. The new translation(s) will and can only be found at, or available via the ild care foundation website, ildcare.nl.
4. PDF is in lay out of the ild care foundation (see website versions: https://www.ildcare.nl/wp-content/uploads/2022/02/679743_FAS_English.pdf).
5. No costs will be charged to the ild care foundation, nor to persons involved with the ild care foundation for any of the translations you will perform.
